Step 1: Click Menu on the top right corner of the page -> Licensing Appointments -> Appointments ->

Carrier Appointments

STEP 2: Click “Fixed/ Index Annuities” –Non-NY Annuity Carriers & apply for the following:

• Allianz - Crump (P00418)

• Nationwide - Crump (P00471)

• Transamerica Life Ins – CRUMP (P00394)

STEP 3: Go back to carrier appointments; click “Life & Disability Insurance” – Non-NY & apply for the following:​

• Everest - IA American (P00435)

• Nationwide (PM0100)

• Pacific Life (P00127)

• Transamerica Premier Life Insurance Company - TPLIC (P00002)

• Transamerica Life Insurance Company TLIC (P00401)

STEP 4: Go back to carrier appointments; click “Long Term Care” & apply for the following:

• Transamerica LTC – Crump (P00475)
